Job summary

The position involves supporting the delivery of care to individuals by performing routine assessment tasks and assisting in various aspects of health and well-being programs. The role requires effective communication with both patients and the care team, ensuring patient safety, dignity, and well-being. It also involves personal development, contributing to the maintenance and development of services, and adhering to quality standards. Main duties of the job

Communication and Reporting:

  • Establish and maintain effective communication with patients and team members regarding routine operational matters.
  • Accurately record information and relay it to the relevant team members in a timely manner.
  • Obtain consent from patients or carers before performing care tasks.

Patient Care and Support:

  • Assist in delivering care programs tailored to meet individual health and well-being needs.
  • Undertake routine assessment tasks, monitoring any changes in the patient's condition, and report them promptly to the team.
  • Prepare and support patients appropriately during care activities, ensuring their privacy, dignity, and personal beliefs are respected.
  • Quickly alert the team to any health or well-being changes or potential risks.

Health, Safety, and Security:

  • Assist in maintaining a safe and secure environment for both patients and staff, adhering to health and safety protocols.

Quality and Equality:

  • Ensure that your actions contribute to maintaining quality care and support equality, diversity, and patient rights.
  • Follow delegated tasks accurately, contributing to the overall quality of services.

Professional Development:

  • Engage in personal development activities to improve skills and knowledge, contributing to the improvement of service delivery.

This role is crucial in maintaining the high standards of care, safety, and patient respect, while also actively supporting the well-being of the team and the individuals in care.
