Job summary

This is an exciting opportunity for a Community Nursery Nurse / Early Years Practitioner working within the Health Visiting Team across HMR.

The role is to promote the health and wellbeing of individual children, young people, families and communities using an early help and partnership approach at every opportunity.

The post holder is required to:

  • Contribute to the delivery of the Healthy Child programme for children 0 to 5 years and their families / carers as delegated by the Health Visitor / Health visiting team lead.
  • Work under the supervision and direction of the Health Visitor/Health visiting team lead.
  • Work with targeted families providing advice and support in response to identified health, social or special needs within the home and other venues.
  • Undertake and assist with the planning of developmental assessments.
  • Be responsible for their own work ensuring effective liaison and regular updates to the Health Visitor / Health visiting team lead.
  • Work innovatively, embracing new developments including the use of technology
  • Plan and implement health promotion activity both for individuals and groups within the local and national Public Health agenda and according to identified need
  • Contribute to locality teams' performance and data capture

Main duties of the job

The Community Nursery Nurse/ Early Years Practitioner works within the Health Visiting Team and supports the team to deliver the Healthy Child Programme for children 0 to 5 years and their families. As member of Rochdale Care Organisation and Rochdale Borough's Integrated Children's Services they will contribute to the expectations of the Boroughs Children and Young Peoples Partnership Strategy and the Early Help Strategy.

  • NNEB or equivalent, eg NVQ Level 3

Knowledge, Training & Experience

Essential

  • Post qualification experience working with children and families in a community setting
  • Understands, and can demonstrate application of, multi-disciplinary working
  • A sound knowledge of child development
  • Knowledge of safeguarding children and adult procedures

Desirable

  • Recent experience of working with other agencies including the Voluntary and Community Sector
  • Knowledge and experience of ASQ developmental assessments
  • Evidence of contribution to service developments

Skills & Abilities

  • Evidence ability to work alone and as part of a team
  • Ability to manage the delegated workload and prioritise

Desirable

  • Competent IT skills and use of digital systems, i.e. Systm1, EY app
