Job summary
Integrated care is all about providing better care and support to our patients, users and the people of Salford by coordinating our services more effectively, with the focus on improving the health and wellbeing of people across the city. Aiming to integrate health and social care in Salford by aligning contributions of the multi-disciplinary team to provide older people with the support they need to manage their own care.
With the ongoing development of proactive care in a neighborhood footprint, you would be working to support the community rehab and falls service in delivering therapy interventions to patients in their own homes including within residential and nursing homes.
You will be working in a well-established therapy team supporting Occupational therapists and physiotherapists in delivering rehabilitation and falls prevention interventions.
You will have some previous experience of working as an OT or physio assistant. You will have excellent interpersonal skills to enable you to work effectively as part of a multi-disciplinary team.
Main duties of the job
You will work under the direction of registered OTs and physios to provide therapeutic interventions and review function for patients and their carers' as part of a multi-disciplinary team in a community setting.
Under appropriate supervision, you will participate in a range of duties to support the community rehab and falls service and to develop your skills in order to deliver high quality care.
You will carry out duties which support the delivery of therapeutic patient care and in line with health and social care priorities, communicating effectively across all interventions, respecting confidentiality of information relating to patients, relatives, staff and the organisation.
You will maintain clear, accurate and full patient records including electronic patient records, undertaking other duties as directed by the service lead including carrying out any appropriate administrative tasks.
