Nursing Associate - Hospital and Care at Home (Virtual Ward)

Job summary
This is an exciting opportunity to join the newly formed Hospital and Care at Home (Virtual ward ) service. The service follows different pathways (Frailty, Respiratory, Heart Failure and Infection) to allow patients to be discharged from Hospital earlier to continue their treatment and recovery at home. Patients can also be admitted directly from the community to avoid a hospital admission where possible.
Patients are supported at home by the Hospital and Care at Home team. We can provide virtual monitoring and daily support through home visits or virtually.
Main duties of the job
The post holder will work within the Hospital and Care at Home team to contribute to the ongoing assessments of patients. Providing and monitoring patient care in line with the service pathways as agreed with supervising clinician(s). Monitor the condition and health needs of people within their care on a continual basis in partnership with the wider Hospital and care at Home team.
Establish and maintain effective communication with patient/carers to enable an environment to support therapeutic relationships.
Visit patients at home to review progress against established pathway. Escalate concerns to the clinical team where required. To provide home visits to obtain blood samples when necessary and to set up patients on remote monitoring.
- Contribute to ongoing assessment and monitoring of patients.
- To teach patients how to use remote monitoring.
- Monitor the condition of patients with support of a supervising clinician.
- To provide home visits to patients who may require assistance with virtual monitoring.
- To provide home visits to obtain blood samples when clinically indicated.
