This is an exciting opportunity for an enthusiastic, motivated, and ambitious Learning Disability Nurse to join our Intergrated Learning disability Service.
The postholder will work as part of Communuity learnign Disability team which is part of the wider Learning Disability Service.
the Learning Disability Service is made up of Learning Disability Nurses, Social Workers, Community Assessment Officers, Speech and Language Therapists, Occupational therpaists, Phsyiotherapists and a team of Psychologists and Art Psychotherapists . The successful candidate will be committed to multi-disciplinary working for the holistic benefit of the people we support. Our large integrated service is built on the principles of Building the Right Support, The Allied Health Professionals (AHPs) Strategy for England, Health and Care Act 2022, and driven by the Care Act 2014 statutory duties of improving health, wellbeing, independence, and safety.
Main duties of the job
The sucessful candidate will provide leadership to the Comunity learning Disabiltiy team on a daily basis as well as having own caseload. This is a leadership role so supporting, mentoring and coaching staff are vital skills, you will also be giving advice and guidance to staff in differing backgrounds within the team.
Undertake complex assessments and develop appropriate support plans, this will involve agreeing services to meet assessed need. Evaluate service and provision of support as per assessment support plan. Complete funding request forms for resource panel and agree packages of care within agreed budget remit.
Provide supervision, coaching and mentoring to individual staff members under the direction of the team manager Undertake the practice educator role and provide assessment and support to staff taking students/learners on placement within the Directorate
