Job summary

An exciting opportunity has arisen for a band 3 Health Care Support Worker to join our integrated team within Heywood, Middleton and Rochdale, you will be based at one of our 5 Neighbourhood Teams, you will provide care and support for patients across the community setting of Heywood, Middleton and Rochdale.

As a member of our Integrated Neighbourhood Team, you will have clinical skills and competencies achieved at NVQ level 3 in Direct Care (or working towards/equivalent experience) and further identified training. You will assist in the delivery of high standards of health care supporting patients within their community setting by following prescribed packages of care.

Main duties of the job

The Band 3 Health Care Support Worker within Integrated Neighbourhood Team will provide care and support for patients across the community setting of Heywood, Middleton and Rochdale. Will participate in the delivery of care delegated without direct supervision, reporting to a Registered Practitioner.

The post holder will be a member of the Integrated Neighbourhood Team, with clinical skills and competencies achieved at NVQ level 3 in Direct Care (or equivalent experience) and further identified training. They will assist in the delivery of high standards of health care supporting patients within their community setting by following prescribed packages of care.
