We are looking for an inclusive compassionate leader to join our friendly, forward thinking, Integrated Community Diabetes Service. The post holder will develop our education offer with innovative delivery models considering health inequalities, inclusion, and neighbourhood ways of working. There will be close working with primary care networks, health, social care and voluntary care agencies.
Main duties of the job
The main purpose of the job:
- Responsible for the day-to-day leadership and management of the Integrated Community Diabetes Service across Bury, Heywood, Middleton, and Rochdale
- Participating in clinical workload such as clinician led clinics, multi-disciplinary clinics and joint diabetes specialist nurse and diabetes specialist dietitian clinics
- Lead role in delivering patient and professional diabetes education within the community, primary care, health, and social care setting. Developing new and innovative ways to deliver education and assisting in the implementation of policies, national, local diabetes care strategies and guidelines.
- To represent the integrated community diabetes service at Greater Manchester Network, Primary Care, and other relevant meetings
- Working as part of the diabetes leadership team
- To work as part of a multi-disciplinary team and autonomously providing high quality care and highly specialist advice to patients with diabetes and their carer's.
- To have a professional lead focus for education within the integrated community diabetes service and represent at relevant meetings and networks
About us
The Integrated Community Diabetes Service which are based at Moorgate Primary Care Centre, Bury and The Phoenix Centre, Heywood provides specialist clinical diabetes care across Bury, Heywood, Middleton, and Rochdale supporting primary and secondary care services. We provide specialist advice, education, and information to support patients in the management of their diabetes, and also support carers, other health professionals and the voluntary sector across a range of community, health and social care settings.
You will work across diverse, and busy community settings to develop and deliver high standards of quality, patient centred and evidence-based care.
Applicants must be a registered NMC/RCN or a registered dietitian with the health & care professional council (HCPC).
Current experience of working as a specialist diabetes nurse or diabetes specialist dietitian in adults is essential together with demonstrable leadership and management experience and a passion for education. Completion of the V300 prescribing/ supplementary prescribing (dietitian) or willingness to undertake this qualification is a requirement.
Working hours are negotiable, minimum of 3 days & compressed hours considered with flexible working opportunities.
