Job summary
Fairfield General Hospital is recruiting to a new and developing In-Patient Discharge Unit
If you are motivated, enthusiastic, and passionate about providing patient centred care, then ward 24 is for you. We are a 24 bedded discharge unit, with a seated area for discharge lounge patients on the day. We are dedicated to getting people home by focusing on preventing deconditioning and reducing the number of days patients are kept away from home. We provide individualised discharge planning. Our ward ethos is to get people out of bed, washed, dressed and ready for the day. We engage in ward-based activities, such as arts & crafts, playing cards, painting nails, taking patients for a walk, and promoting independence as much as possible. This is a team-based approach and requires excellent communication skills, as well as the ability to engage with people from all walks of life and the multi-disciplinary team. The post holder will work as part of a team to provide a safe, effective, efficient care environment for patients, relatives, and carers. They will be responsible for the assessment, planning, implementation, and evaluation of programmes of care under indirect supervision from senior staff. They are expected to be able to demonstrate procedures and supervise unqualified staff, and work without direct supervision.
Main duties of the job

- Establish and maintain effective communication with patient/carers and families about complex matters providing a warm welcoming and enabling environment to support therapeutic relationships
- Develop specialist knowledge, in area of practise, underpinned by theory
- Assess, plan, implement and evaluate the effectiveness of nursing care and adjust the programmes of care where indicated along with maintaining the health, safety and security of self and others in own work area
- Contribute to the development of services
- Contribute to quality improvement
- Support patients, carers and staffs equality, diversity and rights
- Lead others in the development of knowledge, ideas and work practice
- Plan, allocate, assess and provide feedback to team members
- Maintain and support the efficient use of physical and /or financial resources
- You will practice in accordance with the professional, ethical and legal framework for nursing standards
- Ability to work flexibly and unsocial hours to meet the demands of the service
