Job summary
The post holder will work as part of the multi-disciplinary team to provide high quality , functional, holistic assessments to patients to prevent unnecessary hospital admission and facilitate safe, effective discharge to the most appropriate environment under the 'home first' model. The post holder will be expected to work across A&E, Intermediate Care and Crisis Community Enablement Team. They will work in line with the agreed strategic objectives of Oldham Care Organisation.
Main duties of the job
- To work as an autonomous practitioner and be professionally and legally responsible and accountable for all aspects of work including the management of patients in your care in accordance with trust and professional guidelines.
- To ensure a high standard of clinical care for patients with appropriate support from senior therapists and to support more junior staff to do likewise.
- To practice within the Code of Professional Conduct of RCOT
- Assess the functional abilities of the client group, taking into account physical, psychological, environmental, social, behavioural, spiritual and cultural and carer needs. These needs may be complex, changing and vary across the caseload.
- Take responsibility for own caseload and for acceptance in line with the departmental policy and professional guidelines. Provide therapy intervention with support from the Team Leader as necessary. This includes the assessment and provision of equipment/ aids and adaptations, including demonstration, fitting and instruction on the safe use of equipment including risk assessments.
- Full driving license is an essential requirement for this post.
