Job summary

We have an exciting opportunity for a band 6 nurse in Rapid Response and Hospital at Home. There is then a fantastic promotion structure which can lead to Advanced Clinical Practitioner or Management. It is a full multi-disciplinary team, incorporating both health and social care which enhances all skills. Bury is making significant investment to improve the capacity and capability of intermediate care and related services under the leadership of the Bury Integrated Care Partnership [ICP]. The Bury ICP is overseen by a Board with membership from Bury Council, the NHS, the voluntary sector and wider partners.

The shifts for these positions are:

2 x Band 6 Nurses 08:00-20:30 7-day working

2 x Band 6 Nurses 20:00-08:30 7-day working

Main duties of the job

The LCO is working with local Commissioners to deliver the integration and transformation of local health and care services as part of Bury's Locality Plan. This includes plans to enable more people to have their health and social care needs met at home and in the community andavoid unnecessary hospital or care home admission.

The new delivery model for the Intermediate Tier of services is designed to bring together health and social care into a Bury-wide integrated team delivering intermediate care services under single leadership. The key deliverables are to:

  • Introduce home-based care into Bury's intermediate care offer;
  • Create additional enhanced Intermediate Care [IMC] beds through the dual registration of Killelea House;
  • Develop a multi-disciplinary Rapid Community Response Team
