Job summary
The post holder will work in Intermediate Care across two sites in Oldham (Butler Green and Medlock Court) working as part of a multi-disciplinary team on a daily basis. They will also be a member of the Community Services Pharmacy team at the NCA and be managed by the Lead Pharmacist - Community Services. Members of the pharmacy community team across the four care organisations comprising the NCA meet on a regular monthly basis and are working to deliver joint objectives across all IMC services. The post holder will be part of this team and also the wider NCA Pharmacy Team.
Main duties of the job
As an experienced pharmacist, the post holder will work within their professional boundaries to provide Clinical Pharmacy advice to the multi-disciplinary teams within IMC and Integrated neighbourhood teams.
They will conduct Clinical Medication review to proactively manage patients with complex polypharmacy including frail elderly and vulnerable adults to prevent drug misadventure and reduce admission to hospital.
They will work within Intermediate care to support neighbourhood teams to help patients to manage their medication safely, improve compliance, reduce wastage and reduce anxiety following hospital admission.
They will integrate with the wider healthcare teams, including Community and Hospital Pharmacy and Pharmacists working in GP practice, the Primary Care Networks and with other members of the Neighbourhood teams.

- Vocational master's degree in pharmacy or equivalent
- Evidence of an ongoing commitment to CPD1
- Substantial experience in more than one sector of pharmacy
- Experience of implementing change
