We have an exciting opportunity to join our Healthy Futures team as the Specialist Nurse for children with complications due to excess weight. This is an innovative service, funded until April 2027 by NHS England.

The Healthy Futures team are a dedicated Multi-disciplinary Team delivering a high standard of care to these children and their families. They provide specialist outpatient support to children and their families. The role includes training and career development in this specialty as well as leadership and working within the Multidisciplinary team. Ensuring that patient safety is at the heart of everything we do. We are looking for someone who is passionate about patients focused care, with excellent communication skills who is willing to deliver high quality care as part of this team.

Main duties of the job

The successful candidate will work with the Healthy Futures team as well as the rest of the Specialist Nurse team. Patient care will be delivered in mainly outpatient settings as well as providing telephone advice and community support. We would also expect the candidate to undertake relevant training to extend their knowledge and skills in these areas as well as engaging in education for wider clinical staff.

The post holder will be involved with the patients throughout their journey from referral to discharge and will work closely and collaboratively with the wider team, patient and their family to achieve the best outcome.

Job responsibilities

  • To manage a specified case load of patients, providing information and support, to enable patients to make informed choices about their care and understand the rational of investigations and treatments.
  • To work closely with the Consultants, medical and nursing staff, multi-disciplinary team and Heads of Department within the Directorate.
  • To ensure that patients are seen within recommended time scales and targets where these are in place.
  • To visit inpatients as appropriate to ensure continuity of care and to give advice/ support.
  • To accept referrals from medical/nursing staff both within and outside the Trust, of newly diagnosed and existing patients
  • To periodically review caseload and to discharge patients appropriately within divisional guidelines
  • To respond to telephone enquiries and co-ordinate admission or changes in appointment dates as warranted by the patients clinical condition, including operation of a telephone helpline for the relevant speciality if required.
  • To undertake a comprehensive assessment of the patients symptoms and in liaison with the Medical team organise investigations as required.
