Administration Assistant - Integrated Discharge Team

Job summary

An opportunity has arisen to join the Integrated Discharge Team, as Band 3 Administration Assistant

The post holder will be responsible for supporting the Integrated Discharge Team in providing comprehensive and efficient collection, analysis and input of high-quality patient data and administrative support. To provide an essential support service to the Integrated Discharge Team ensuring the smooth running of the Team, the patients discharge journey, impacting on hospital flow and performance.

The post holder will work closely with the Home First Hub, and system partners supporting with processing and uploading of referrals, monitoring the Discharge Hub Mailbox and disseminating communications to appropriate team members, as well as providing accurate and timely reports.

Main duties of the job

The post holder will be expected to monitor the discharge hub inbox, keeping up to date with correspondence, upload referrals, ensure our trackers and spreadsheets are updated, and to complete daily discharge data and information to assist the Integrated Discharge Team in sustaining the work of the team.

The post holder will need to have good communication skills, be organized and have an attention to detail, there will be an expectation on behalf of the Integrated Discharge Team to speak with other departments to share and resolve queries relating to patient discharge.

This role requires excellent organisational skills and the ability to work both independently and collaboratively, with an ability to use your own initiative and work under pressure.

  • The collection and interpretation of comprehensive and accurate data for the Integrated Discharge Team.
  • Manage data and information to assist the Integrated Discharge Team in sustaining the work of the team, which will contribute to the overall improvement of Discharge Services across the whole Organisation. Extraction of data from PAS, ICE, Optica and SystmOne
  • Monitor the Discharge Hub Outlook Box keeping up to date with correspondence within the integrated Discharge Team in a timely manner.
  • Process referrals from current referral system (ICE at time of writing) and accurately record and manage referred patients Optica entries.
  • Complete all elements of the covid Discharge Situation Report daily and send to NHS England.
  • Ensuring the Patient Tracker spreadsheets are up to date: daily use entries in addition to totalling referral and discharge tables and Fast Track Outcome sheets
  • Completing the daily Discharge Summaries.
  • Support with Out of Area Referral forms, liaising with Out of Area Trusts and Social services as required.
  • Respond to ad-hoc requests from senior nursing and medical staff for information related to quality and improvement schemes within the Integrated Discharge Team.
