Are you enthusiastic, compassionate, self-motivated and want to develop your skills in Specialty care? If so, this is the post for you! We are looking for a Specialist nurse for Children with Medical Complexities to join our team.

This is a really exciting post that will be right for someone who has a passion for delivering joined up, supportive and high skilled care to children with medical complexities and their families. Also, the post holder will have the opportunity to develop and demonstrate abilities in strategy, service development and improvement.

The successful candidate will work with specialist and clinical teams as well as the Roald Dahl Marvellous Children's Charity to develop and deliver a well organised and responsive service to this patients group. There will be access to a wide variety of training opportunities from both the NNUH and the Roald Dahl Marvellous Children's Charity (RDMCC), where they will find a supportive community of other CMC Nurses to share leaning and experiences with.

Main duties of the job

The successful candidate will work with both clinical & specialty teams at the NNUH to identify & support a case load of patients with the highest medical needs. They will provide both highly clinically skills care as well as holistic care to the patients & their families though admission planning, acute/elective presentations to the NNUH & through complex discharge coordination.

The post holder will also have access to a wide variety of support & training provided by the Roald dahl Marvellous Children's Charity to both support their learning & the families on the case load.As this is a new service, we are looking for someone with good experience of change management & service improvement work who can continue to develop this role & present progress both to the NNUHFT & to the RDMCC. Therefore, we are looking for someone who is self-motivated with good organisation skills. Also, someone who is happy to present & feedback to both the NNUH & RDMCC to progress of the service.

Job responsibilities

The role will involve working with professionals from a wide variety of professional backgrounds as well as patients and their families. We are looking for someone with good experience of caring for this patient group, and good communication skills to be able to bring families, patients and professionals together to deliver the holistic, joined up care we are striving for.

  • To operate autonomously at an advanced level of clinical practice providing research based expert care and advice to all colleagues and managing a caseload effectively, making the most of available time
  • Work collaboratively with all stakeholders including MDTs, CNSs, clinical teams, Consultants, therapists, and community nursing teams across the Trust/Health Board to facilitate improved patient flow, capacity and continuity of care.
  • To accept referrals from medical/nursing staff both within and external to the Trust, of newly diagnosed and existing patients
  • To ensure that patients are seen within the recommended time scales and targets where these are in place
  • To utilise specialist, skills, underpinned by in depth theoretical knowledge, to facilitate the Co-ordination of multidisciplinary assessments meeting the physical, psychological, emotional and spiritual needs of the CYP and their family at times of stability and crisis and together with the multidisciplinary team develop strategies to address these needs.
  • To respond to telephone enquiries and assess patients via telephone communication to co-ordinate admission or changes in appointment dates as warranted by the patients clinical condition, including the operation of a telephone helpline for the relevant speciality if required
  • Prepare and participate in patient reviews and child protection case conferences with team members and other agencies and providing reports as required.
  • To supply and/or administer medications according to Patient Group Directions or in accordance with the Trusts Prescribing Policy for registered nurse prescribers.
  • To work closely with own team, Clinical Director, Senior Nurse, Service/Operational Manager, Assistant Director of Nursing and Roald Dahl CMM organisation to maintain professional relationships with patients, relatives, and all members of the multi-disciplinary team.
  • Make judgements for patient management through analysis of available information and initiate referrals to multidisciplinary specialists as judged appropriate.
