Job summary

Fixed term contract/secondment opportunity for 9 months

The Oncology and Haematology Directorate consists of Outpatients within the Colney Centre, Mulbarton Ward and Chemotherapy and Radiotherapy treatment areas. Paperless processes are in place for chemotherapy treatments and in development for radiotherapy with a dedicated software system including electronic paper records. The department has a year on year increase in new referrals and treatments and has recently expanded the radiotherapy and chemotherapy treatment areas.

Part time: 30 hours per week

Main duties of the job

This is an exciting opportunity for an enthusiastic individual to join the Haematology department as the Patient Pathway Coordinator (PPC).

The successful applicant will provide consistent patient-centred focus throughout the pathway of care for patients on a Haematological cancer pathway, whilst ensuring to capture Cancer Waiting Times data for the Trust and Cancer Registry. The successful applicant will work closely with their MDT Coordinator/Clinical colleagues to ensure all relevant patients are meeting their cancer targets by adhering to the 31 day and 62 day targets.

We are looking for a proactive and motivated individual with excellent communication, organisational and interpersonal skills with a great attention to detail.

Ideal candidates will have to demonstrate their experience in organising and prioritising workloads. Successful candidates will receive plenty of support, guidance and learning to carry out the role and will be supported with their learning and development.

Job responsibilities

  • Understanding of, and interpretation of National policies and guidelines in relation to oncological and haematological malignancies and development of service improvement action plan accordingly. Monitoring progress and supporting the team in any service improvement activities.
  • Continuous review and updating of patient pathways.
  • Liaising with the clinical teams, explore new ways of working and where agreed, pilot possible solutions and help to implement approved changes.
  • Manage the co-ordination of the administration pathway for oncology and haematology patients.
  • Manage appropriate communication ensuring 24 hour GP notification standard is achieved.
  • To undertake and support the clinical audit process within Oncology & Haematology, linking where appropriate with the Trust and NWCN audit teams.
  • To attend haematology site specific meetings on a regular basis and advise the clinical lead on any perceived problems relating to MDT working, information and data.
