Job summary

The Virtual Ward is an exciting addition to the way we care for Patients. The ward exists virtually allowing patients to be in their own homes with monitoring and communication with the team 24/7. We also work with existing services to arrange IV's and Phlebotomy services. The daily consultant ward round reviews and plans next steps. Patients enjoy being supported and our Nurses love the challenges of working creatively.

Full time: 37.5 hours per week

Main duties of the job

The Virtual Ward continues to expand in both with numbers we can manage and the types of patients/ pathways. We therefore are looking for staff familiar with the Virtual Ward who can be both resourceful and creative. They must be great communicators so as to explain our new service to patients as well as staff.

  • To be supportive for Ward nursing staff and to provide a quality and cost effective service in accordance with Trust policy and procedures.
  • Ensure that accurate records are kept of all care given in the Ward
  • Act as the key person in the coordination and communication of complex, sensitive, emotional, condition related issues to patients, relatives and to the multi-disciplinary team.
