Job summary
The Senior Anticoagulant Nurse Specialist will lead the nurse-led anticoagulation service in collaboration with the Consultant Haematologists.
The service provided will cover the fast track DVT service
Full time: 37.5 hours per week

Main duties of the job
As the successful post holder, you will:
- act as a resource by providing information, advice and guidance on anticoagulation for patients, their carers/relatives and other health professionals.
- co-ordinate the management of the clinical caseload and will link closely with the primary care team and other health professionals to ensure continuity of care through all health care settings.
- co-ordinate and contribute to the provision of specialist education and training programmes for junior doctors, nursing staff and students in the Trust and for GP surgery staff.

- In conjunction with the oncology/haematology matron lead the provision and development of the anticoagulation nursing service, providing vision and direction and ensuring a high-quality patient-centred focus.
- In conjunction with the oncology/haematology -matron, lead and manage the anticoagulation nurse specialists in delivering clinically effective, high quality patient centred care; maximising the potential of team members and creating opportunities for staff to work effectively.
CLINICAL
- To lead in the effective management and delivery of the nurse-led anticoagulation service as outlined above, working to Trust protocols, guidelines, non-medical prescribing, and patient group directions (PGDs).
- To manage and co-ordinate the assessment and diagnosis of patients referred to the service by assessing, planning, implementing and co-ordinating their care, including prescribing anticoagulants or supply and administer under PGD or drug protocol according to Trust protocols.
- To lead in the development of the service to meet the needs of the patient groups in line with best practice, evidence based nursing and national standards for anticoagulation practice.
