Job summary

Fixed term contract/ secondment opportunity for 10 months.

As a Cancer Care Navigator you will be uniquely positioned to promote self-management and provide general information and support about cancer and cancer services, to enable patients to navigate the health and social care system and make choices that are important to them.

The team provide complementary support to the Cancer Clinical Nurse Specialists in supporting patients through their cancer diagnosis, treatment and beyond.

Key aspects of the role involves daily communication with patients, relatives, carers, and other health and social care professionals.

You will be undertaking Holistic Needs Assessments following diagnosis, signposting to support and information services. The information and nature of communication is sensitive. Communication in this context requires a high degree of empathy, understanding, diplomacy, confidentially and integrity.

We are interested in candidates that have or are working in the health and social care system and able to demonstrate:

  • an ability to deal with complex and difficult emotional situations
  • have an awareness of the health and social care environment
  • have good IT skills that can cross different NHS IT systems
  • have effective organisational skills
  • have good communication skills.

Main duties of the job

You will be a self-motivated and enthusiastic individual with experience and understanding of the health and social care environment. You will be supported by the wider Cancer Care team in your ongoing development and learning to deliver seamless and compassionate care.

You will co-ordinate care with the person living with cancer so that they access the right help at the right time, providing a seamless experience of care. Checking that the person can self- manage and provide a flexible and personalised response to any changes needed. You will help people living with cancer to access referrals and/or signposting to local supportive services that can meet an individual's range of personal needs.

Good and accurate IT skills will help you to record confidential information by completing databases, templates and process flows to ensure patient information is shared in a timely and effective manner as well as provide updates to allow the ongoing evaluation and impact of the team.

Record all patient contacts with Somerset Cancer Registry Support people to access local appropriate information and sign-posting to a range of support services, an approach which helps people to self- manage where appropriate. Advise patients on individual self-care management principles and provide consistent planned aftercare to reinforce and further promote this information. Have up to date knowledge and links with the breadth of services available to support patients self-management across North Suffolk and Norfolk To have the skills and training to assist patients and families to access Personalised Care. To be a key contact for primary care teams to link the patient journey between acute and community setting. Have undertaken appropriate training to support the development of and completion of care plans, holistic needs assessment, HOPE course, and other self-management/recovery package related services to ensure a consistent and quality service. Create, develop and maintain efficient and effective Microsoft Office Systems e.g. Word, Excel, Microsoft Project essential for the smooth running of the Navigator service.

You will also be required to support our Macmillan Information Managers at our Pod/Centre at both NNUH and Cromer and Health and Wellbeing Events across the locality.
