Health Care Support Worker - Band 3 - Part Time 22.5 Hours per Week - Summerview Rehab Unit:

We are seeking to recruit a positive, motivated, and compassionate Health Care Support Worker who is committed to the principles of recovery, to work as part of the multidisciplinary Rehabilitation Team based at Summerview Rehabilitation Unit.

This exciting opportunity will enable the successful candidate to gain experience of supporting people to develop, retain or regain independent living skills.

Main duties of the job

To provide support, and spend therapeutic time with an allocated group of service users.

To promote and support the physical health, comfort, wellbeing and recovery of service-users, and to assist them in re-establishing their place in the community, as appropriate.

To assist Registered Professionals in assessments and care planning/ implementation and evaluation of individualized care plans.

To provide support and day-to-day supervision to junior staff members, demonstrating the Health Care Support Worker role and role modelling good practice.

Job description

Job responsibilities

1. Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.

2. Regularly reports and effectively communicates service-users health status and care aims in care records and verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team. (query blending the above two first is about patients/public, second is healthcare staff)

3. Relates appropriately with service-users, carers, colleagues and others at all times maintaining professional boundaries.

4. At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals.

5. Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues.

6. At all times minimizes risks to self and others by undertaking safe working practices.

7. At all times works within agreed and established Trust policy and operational procedures.

8. Works with other Trust services to promote seamless service-user person centred care from referral/admission, to transfer and discharge on identified care pathways.

9. Responsible for contribution towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users.
