Job summary
We are seeking amotivated and compassionate Healthcare Support Worker to join our team and help deliver high-quality, person-centred care. This is a substantive post that plays a vital role in supporting patients and the wider multidisciplinary team.
£24,937 - £26,598 per annum, band 3, full time 37.5 hours per week.
You will contribute to meeting safe staffing levels and ensuring patients receive timely, respectful, and effective care.
Main duties of the job
To provide support and spend therapeutic time with an allocated group of service users.
To promote and support the physical health, comfort, wellbeing and recovery of service-users, and to assist them in re-establishing their place in the community, as appropriate.
To assist Registered Professionals in assessments and care planning/ implementation and evaluation of individualized care plans.
To provide support and day-to-day supervision to junior staff members, demonstrating the Health Care Support Worker role and role modelling good practice.

KEY DUTIES AND RESPONSIBILITIES:
The post holder will undertake the following duties and responsibilities at all times demonstrating the competences assigned to the role detailed in the associated KSF outline:
1. Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
2. Regularly reports and effectively communicates service-users health status and care aims in care records and verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team. (query blending the above two first is about patients/public, second is healthcare staff)
3. Relates appropriately with service-users, carers, colleagues and others at all times maintaining professional boundaries.
4. At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals.
5. Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues.
6. At all times minimizes risks to self and others by undertaking safe working practices.
7. At all times works within agreed and established Trust policy and operational procedures.
8. Works with other Trust services to promote seamless service-user person centered care from referral/admission, to transfer and discharge on identified care pathways.
9. Responsible for contribution towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users.
10. Responsible for undertaking elements of a holistic assessment, under the direction of Registered Health Professional staff. Responsible for regularly working with service users to negotiate and review existing plans to address identified needs under the supervision of Registered Professionals.
11. Works in partnership with service-users to assist them in identifying activities that are meaningful to them and that support agreed health outcomes.
12. Regularly supports service-users to access a range of activities and services to maintain their independence and promote their health and wellbeing.
13. Regularly provides advice and guidance to Service-users promoting healthy activities and lifestyle choices.
14. Regularly supports Registered Practitioners and Service-users during activities to meet their physical and mental wellbeing.
15. Regularly supports service-users in meeting their assessed personal care needs.
