The Specialist Adult Eating Disorder Service, SAEDS, are looking to recruit a part time [30 hours] Band 3 HCSW.
The post holder will work across the All Age Eating Disorder Service working with Adults, young people and their families to deliver outstanding care, helping to promote independence and dignity and positively enhance the lives of individuals in our care. The post holder will be expected to help practically, supervising meal times in a variety of environment, liaising with schools, colleges and families offering psychoeducation around food, eating disorders and manging emotions. You will be part of resource development, advice promotion to other professionals.
Main duties of the job
Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals.
Identifies, minimizes and appropriately reports risks of danger, harm and abuse.
Works with other Trust services to promote seamless service-user person centered care from referral/admission, to transfer and discharge on identified care pathways.
Responsible for undertaking elements of a holistic assessment, under the direction of Registered Health Professional staff. Responsible for regularly working with service-users to negotiate and review existing plans to address identified needs under the supervision of Registered Professionals.
Works in partnership with service-users to assist them in identifying activities that are meaningful to them and that support agreed health outcomes.
