Job summary

An exciting opportunity has arisen for a full time Band 4 Support Time and Recovery Worker to work in partnership in Primary Care as part of the Additional Roles Reimbursement Scheme (ARRS). The post sits within the Meir Primary Care Network MDT working alongside the Senior Mental Health Practitioner and GP surgery teams.

You will be responsible for providing mental health specialist care to patients within the PCN, whilst working as part of the network's multidisciplinary team.

Meir Primary Care Network brings together six GP practices that work closely to support people across our community. We focus on practical, joined-up care and good access to the right help at the right time. Our teams include clinicians and a wide range of additional roles who work across practices every day. People know each other, support each other, and share a commitment to making care easier for patients.

As a Support Time Recovery Worker within Meir PCN, you will be part of a small, approachable team that values steady, patient-focused work. You will help people build confidence, stay connected with local services, and make progress with their personal goals. You will have regular contact with GPs, care co-ordinators, social prescribers and your ARRS mental health colleagues who are ready to guide and support you.

Main duties of the job

You will engage with patients to provide support and time limited interventions to meet their health and social needs either through individual or group work with an emphasis on empowerment and recovery. Working closely with the Mental Health Practitioners to devise plans of care to support recovery and advocate improved choice and access to evidence based mental health and social care support - using a strengths and asset approach focusing on harnessing and improving patients own network to improve resilience.

Experience of working in the community in a mental health setting (either in CYP, Adults, and Older persons) would be beneficial. Primary care experience is not essential; however, a good understanding of how Primary Care works would be beneficial.

Most importantly, the right candidate will have a strong desire to achieve the best possible outcomes for patients and have the motivation and flexibility to help develop the services as the role evolves to meet the needs of the population.

The role will be embedded within the PCN however it will be outward facing and will link seamlessly with North Staffordshire Combined Healthcare Trust where you will be supported with supervision, training opportunities etc.
