The Sutherland Centre is a busy, fast based adult community mental health team within Stoke on Trent. We are a large, supportive team who take pride in our sense of team and prioritise development and staff wellbeing. The local CMHT's are constantly changing and developing, in order to meet the needs of our local community, and so a willingness to work with change and have a positive mindset is essential. We are looking to recruit a full time B6 Community Mental Health Nurse.
The successful candidate will:
- Work within a fast paced, supportive multi-disciplinary team as a keyworker for a specific group of clients
- Provide care and therapeutic intervention which might include assessments, 1:1 or group based interventions; depots and physical health based tasks, management of complex SMI patients
- Caseloads will be varied and you will be offered supervision from the caseload manager/clinical lead
- You will be required to liaise with other professionals, agencies both statutory and non-statutory, to provide a link between hospital and community and/or primary and secondary services and attend MDT meetings regarding progress and outcomes.
- Ensure that practice remains abreast of new developments and to develop and maintain standards and implement change in respect of this
Main duties of the job
To Act as named nurse for a specific group of clients and provide care under the supervision of the caseload manager. To conduct assessments and to deliver specific therapeutic interventions to this group of clients and to liaise with other professionals regarding progress and outcomes.
To ensure that practice remains abreast of new developments and to develop and maintain standards and implement change in respect of this.
To liaise with other professionals and agencies both statutory and non-statutory and to provide a link between hospital and community and/or primary and secondary services.
To operate a duty role in the screening of referrals and allocation for assessment. To provide support and guidance to colleagues over issues of risks and/ or complex clinical presentation To act as named nurse with responsibility for the assessment, planning and delivery of care to the identified patient group.
