If are you an experienced & resilient clinical practitioner within mental health services, then look no further.This is a great opportunity to join our Crisis Resolution and Home Treatment Team (CRHTT) as a Senior Mental Health Nurse.Our CRHTT is part of the Crisis Care Centre, at Harplands Hospital, Stoke-on-Trent which has been designed to the highest level of specification. This modern purpose designed unit is the central point for all those in need of a Crisis Assessment/Intervention.

Main duties of the job

To carry out mental health assessments including risk to service users that are having first contact with Mental Health services that are at the point of crisis / or suffering a deterioration and decline in their mental illness. As a band 6 nurse you will be expected to take the lead in decisions and supervision and guide more junior staff. The role will include you acting as a duty lead and organising work and prioritising work to other team members. As a band 6 nurse you will be seeing services users that are referred to services either in the unit or at home to assess for mental health needs. You will work closely with the service user to provide them with the right pathway for them to follow.

Main Duties and Responsibilities:

1. To assess and manage mental health needs, including risk, to create care plans with the individual that will lead to recovery whilst supporting them with a safety plan.

2. To act as the duty lead on shift and allocate work to other team members as appropriate, to run the shift in a safe and effective way.

3. To manage and prioritise work that is allocated to you, to complete the assessment of mental health and risk and record on electronic systems as needed.

4. To be able to supervision and caseload mange more junior staff.

5. To take referrals over the phone, via fax or by post.

6. To work in conjunction with other professionals E.g. GPs, Police, Ambulance, Housing

7. To works in partnership with the individual to develop a plan of care that the service user agrees with.

8. To review the effectiveness of the care plan where appropriate.

9. To make a speedy and appropriate response to all work referred.

10. To maintain client records in accordance with Departmental requirements and to ensure that they are properly stored and available to the Line Manager at all times, including the appropriate use of Information Technology.

11. To undertake regular supervision or consultation with the Line Manager and to ensure that the Team Manager is made aware of any issues of major importance or concern.

12. To carry out such other duties as may be required from time to time and which are commensurate with the grade of the post.

13. To develop good working relationships with other agencies and other colleagues.

14. To promote anti-discriminatory and non-oppressive practice and service delivery.

Qualifications

Essential

  • Recognised qualified i.e. Registered Mental Health Nurse at either Diploma/Degree level To be registered with the NMC.

Desirable

  • To be able to prescribe medication Safeguarding level 3 Ability to use electronic patient records system E.g. Lorenzo

Experience

Essential

  • Experience of assessment and care planning within either a Health or social care setting, including an understanding of needs led assessments Experience of community based Mental Health services and other health and social care services Experience and achievement in working at a multi-agency level Experience of reviewing care plans and monitoring care arrangements. Relevant experience in either a paid or voluntary capacity of working with adults.

Desirable

  • Experience in physical health assessments. To be able to resolve a problem with a positive and safe outcome.
