Clinical Team Manager

Job summary

An exciting opportunity has arisen for a band 7 Clinical Team manager to lead our HVU team.

The key aim of the service is to reduce the number of unplanned attendances to the acute hospital by facilitating and coordinating multidisciplinary support and intensive, time-limited interventions to patients who frequently attend A&E. High intensity users of A&E departments have been identified as having a high impact on the wait times and flow of the hospital, other emergency services and health systems.

The service will ensure the coordination of care and the provision of an assertive outreach approach to vulnerable groups of patients in the community, who typically disengage with mainstream services and / or have a disproportionate use of emergency services (acute hospital and ambulance services).

We work with other health and social care professionals which may include GP, MH services, social care and many more ensure the patients are accessing appropriate services to meet their needs and to formulate a single personalised care and risk management plan.

You will be responsible for a small case and to provide day-to-day oversight of operational and clinical management. We are looking for someone who will be enthusiastic, self-motivated and have excellent communication skills. The post holder will work closely with the UHNM and must be able to work collaboratively with all system partners.

The team operates Monday-Friday, 09.00-17.00 and are predominately home based.

Main duties of the job

The High-Volume User Team is also an essential component of the crisis care pathway supporting our healthcare system. We support individual's attending physical health care settings, namely Accident and Emergency with a combination of physical, psychological and social factors. We offer a community based holistic, assertive outreach pathway to ensure individuals have access to the care and support that meets their needs with a view of reducing avoidable A&E attendances.

Provide management and clinical leadership to the multi-disciplinary team to ensure care is delivered which is person centred and meets standards for Quality and Safety, in partnership with service users and carers.

The post holder will lead from the front & support the team to reach its full potential. Supporting all members of the MDT to develop & ensuring service users receive the highest standards of care during their journey through the service.

The post holder will be a key team member in the wider rehabilitation MDT, working closely with colleagues from our, The Royal Stoke Hospital, CMHT's & external providers such as Drug and Alcohol Service and The Local Authority.
