Job summary

This is an exciting opportunity to join the Memory Services Team for Combined Health Care NHS Trust. Our Trust has been awarded an outstanding rating via the Care Quality Commission (CQC). Memory services are accredited via the Memory Service National Accreditation Programme (MSNAP). Our Memory Service is part of Community Directorate and Older Adults services. We work closely with the local council and Alzheimer's Society and support a working strategy for Dementia across Staffordshire and Stoke On Trent.

Main duties of the job

The Memory Clinic Practitioner will be part of a team that undertakes assessment and diagnosis of a dementia. As part of the role we have links services both statutory and voluntary to support a wider dementia care pathway. The applicant will work in both a clinic and community setting and will liaise with services to support people, families and carers affected by dementia. As part of our work we have opportunity to learn more about dementia via forums, on line learning and conferences which enhance the experience of the practitioner to improve our pathway and care provision.

About us

The Trust offers full Mandatory training and training will be given to Tier 2 Dementia Training.

The hours will be Monday to Friday 9-5pm. This role would be suitable for an enthusiastic Mental Health professional with working with dementia experience or a strong interest in this area. The applicant will have good communication skills and be able to offer sensitive support, knowledge, experience further working with Primary Care Networks. The post holder will have a keen interest in working with Dementia through all stages working within our pathways.
