Band 3 Health Care Support Worker - All Age Access Team, Harplands Hospital:

Our All Age Access Team is part of the Crisis Care Centre, at Harplands Hospital, Stoke-on-Trent which has been designed to the highest level of specification. This modern purpose designed unit is the central point for all those in need of a Crisis Assessment/Intervention. Teams within the Crisis Care Centre include the All Age Access Team, Home Treatment Team, High Volume Users Team and a commissioned place of safety for those detained on a section 136.
Main duties of the job
To provide support, and spend therapeutic time with an allocated group of service users.
- To promote and support the physical health, comfort, wellbeing and recovery of service-users, and to assist them in re-establishing their place in the community, as appropriate.
- To assist Registered Professionals in assessments and care planning/ implementation and evaluation of individualized care plans.
- To provide support and day-to-day supervision to junior staff members, demonstrating the Health Care Support Worker role and role modelling good practice.

- Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
- Regularly reports and effectively communicates service-users health status and care aims in care records and verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team. - Relates appropriately with service-users, carer's, colleagues and others at all times maintaining professional boundaries.
- At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals.
- Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues.
- At all times minimizes risks to self and others by undertaking safe working practices.
- At all times works within agreed and established Trust policy and operational procedures.
- Works with other Trust services to promote seamless service-user person centred care from referral/admission, to transfer and discharge on identified care pathways.
- Responsible for contribution towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users.
