We have an exciting opportunity to recruit a band 6 Mental Health Practitioner who has an interest in Adult and Older Persons Mental Health to the HIPC PCN through Additional Roles Reimbursement Scheme (ARRS).
The adoption of this role is an exciting and new development that will build on and complement the progress of the new model of primary mental health care support.
HIPC PCN are a group of practices working together to focus on local patient care. With asmall multi-disciplinary team, covering 5 practices across Stoke on Trent North areas, covering a total population of 42,000. HIPC are a forward thinking, innovative PCN who utilise the skills and experience of team members to the full - within a supportive and friendly environment.
We are seeking to find a Mental Health Practitioner who can come and make a difference. Providing high quality interventions to patients, working alongside other PCN based mental health roles you will be an important member of our primary care team.
The Mental Health Practitioner will be embedded as part of the Primary Care Network Multi-Disciplinary Team. Working alongside the other roles, they will also be part of the community mental health Multi-Disciplinary Team (MDT) with ready access to a wide range of support and expertise across both MDTs.

Main duties of the job
The role delivers timely access to an integrated, high quality, service user-centred service in primary care that enables people with mental health needs and/or social care problems (regardless of diagnosis) to improve their well-being and functionality, thereby preventing, reducing or delaying the need for more specialist services (either for mental health, physical health or social care needs). It is envisaged that the focus of a practitioners work will generally be people whose needs cannot be met by IAPT services but who may not need ongoing care from secondary care community mental health services. The practitioner can facilitate onward access to other secondary care-based services, or third sector services integrated within the mental health pathway.
Successful candidates must be experienced mental health practitioners and have experience of working in a Primary Care and / or a Community Mental Health Team setting.

The post holder will demonstrate outstanding autonomous and critical thinking and is a caring, compassionate and committed experienced mental health professional. They will, whilst acting within their professional boundaries, provide care for the presenting patient from initial history taking, assessment and management of care, including escalation to a more senior clinician. They will demonstrate safe, competent clinical decision-making and expert care for patients within the practice and wider primary care network. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. They will liaise with other professionals and agencies both statutory and non-statutory and to provide a link between primary and secondary services from a North Staffs Combined perspective.
Key Duties/Responsibilities
- To undertake assessments of individual clients difficulties, establishing risk and determining the appropriate course of action taking into consideration all possible options. The relationship should adopt a partnership approach engaging service users in the positive management of their health and wellbeing.
- Proactively identify and manage treatment plans for patients with dementia and serious mental illness.
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Work with patients in order to support compliance with and adherence to prescribed treatments.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, escalating to senior MHP if necessary.
- Take prompt action on changes in condition, liaising with other caregivers, members of the clinical (multi-disciplinary) team and/or other agencies when necessary, reviewing care and communicating any amendments to the medical records.
- To have responsibility for seeking support and advice from General Practitioner or senior MHP/supervisor in respect of issues and difficulties with individual patient care. To attend regular clinical supervision with the senior MHP in order to address caseload management issues.
- Develop skills in respect of specific interventions that are appropriate to the clinical area and act as the lead in developing practice in relation to this.
