In 2021, it extended its services to incorporate the delivery of the Staffordshire and Stoke-on-Trent Transforming Care Partnership programme (TCP) on behalf of Staffordshire and Stoke on Trent ICB (Integrated Care B ord). Geographically it covers North Staffordshire, Stoke, East Staffordshire Southeast, Staffordshire & Seisdon Peninsular, Cannock Chase and Stafford & Surrounds.

As part of this exciting development, a unique opportunity has arisen to invite applications to join the Trust on its journey. We are looking to appoint to two 12 month fixed term band 7 positions for motivated and enthusiastic qualified nurses (primarily learning disability / mental health professionals) to join the TCP Case Management Team. The team was awarded Team of the Year in 2022 and provides a strong values and Trust led culture. (See Job Description for full details).

The Case Management team works within the Trust to support the delivery of Transforming Care across all Trust Divisions, working to ensure that people with learning disabilities and / or autism are supported to lead healthy, valued lives.

Main duties of the job

The purpose of this role is to support the commissioning of safe and effective services for people with a learning disability. This supporting the discharge from hospital and the oversight of commissioned community packages of care thereby preventing hospital admission.

It will involve participating in the assessment, planning and implementation of pro-active care and case management for the population of Staffordshire and Stoke- on- Trent with a Learning Disability and or autism. The post holder will be the main point of contact for health and social care colleagues as well as providers, service users, family and carers in relation to the appropriateness of placements and facilitating the care system to move people through the system with regards to the Transforming Care Concordat.

The post holder will be a member of the Learning Disability Transforming Care Team within the Specialist Directorate of North Staffordshire Combined Healthcare NHS Trust. They will be accountable to the Service Manager and will have designated responsibility for the co-ordination and delivery of the Transforming Care Programme.
