Discharge to Assess Liaison Nurse - Band 6 - Mental Health Liaison Team:

There is an exciting opportunity to work as a Discharge to Assess Liaison Nurse within the Mental Health Liaison Team.

The MHLT is based at the Royal Stoke University Hospital. The team is multi-disciplinary, comprising nurses, social workers, medical staff, a psychologist and an occupational therapist.

We provide psychiatric assessments and short term intervention to patients attending the emergency portals and in-patient wards at the Royal Stoke and at local Community Hospitals; Haywood, Cheadle and Bradwell.

We also deliver training and education to general hospital colleagues in order to improve their knowledge, skills and confidence in the basics of management of the common mental health problems (depression, dementia, delirium, anxiety, CAMHS) that they encounter in their day-to-day practice.

Main duties of the job

The post holder will be play a pivotal role in supporting the health and social care system with the safe discharge of patients from hospital as crucial member of the mental health liaison team (MHLT).

The post holder will be enthusiastic and motivated with experience of supporting enabling care for older adults and able to demonstrate clinical knowledge and skills.

The post holder will be responsible for undertaking person-centred assessments for patients at the Royal Stoke University Hospital (RSUH) who have been identified as potentially suitable for a Discharge to Assess (D2A) pathway or approaching discharge. The post holder would consider the best discharge option for the patient, always considering home as the first option if suitable, as per the Discharge to Assess principles.

The post holder will work closely with the multidisciplinary team on Ward 4 at the Harplands Hospital (which comprises 15 D2A beds) and with the Track and Triage team at RSUH.

The post holder will also require relevant links and understanding of wider mental health and discharge pathways.

The post holder will use their knowledge and skills to support the discharge to assess pathway, this may include relevant assessments, liaising with a number of services, understanding and navigating pathways to support the best outcomes for service users. Being part of the Mental Health Liaison team offers the post holder access to a wider MDT including medical and psychology support to call upon as needed.

Job responsibilities

  • The post holder will be responsible for gathering and recording information to enable accurate and individualised patient assessments to facilitate timely discharge of patients to their own home, care placement or D2A assessment bed.
  • The post holder should be able to prioritise their own workload to ensure effectiveness and efficiency in delivering objectives.
  • To review patients medical information and discuss with relevant teams if needed to identify any potential red flags, liaising with the MHLT medics in the first instance.
  • To provide a clinical handover of information.
  • To assess and clearly document the patients capacity for the decision regarding discharge destination and admission to placement or hospital, if this is indicated.
  • To liaise with patients and relatives regarding discharge plans.
  • To work closely with North Staffordshire Combined Healthcare Trust (NSCHT) discharge teams and to understand ward capacity/gatekeeping requirements for admission and any upcoming discharges.
  • To work closely with partners and act as a link between NSCHT and partners, building effective working relationships
