Senior Mental Health Practitioner - Band 6 - Community Triage Team:
There is an exciting opportunity to work as a Senior Mental Health Practitioner within the Community Triage Team.
The Community Triage Team (CTT) (also known as 'Street Triage') is a collaboration between Staffordshire Police and North Staffordshire Combined Healthcare NHS Trust. It is a small team of designated, experienced mental health nurses and a Police Officer based at Hanley Fire Station, working from 4pm-2am seven days a week.
The CTT aim to ensure that the police have sufficient understanding about someone's needs. They aim to support individuals who come into contact with police outside of custody who are experiencing mental health difficulties or emotional distress. The team provide timely crisis assessment, advice, support, signposting and completion of referrals to address the individual's difficulties, as well as offering assessment in order to divert from Section 136 MHA detention.
Main duties of the job
The post holder is responsible for the delivery of care of a specific group of patients in contact with the Community Triage Team (CTT) under regular supervision of the team manager.
To deliver specific therapeutic interventions to this group of patients and liaise with other professionals both statutory and non-statutory regarding progress and outcomes and to provide a link between all agencies involved in the care and management of CTT service users.
The post-holder will assist in setting and maintaining standards of care; will participate in maintaining effective management of the team. To ensure that practice remains abreast of new developments, maintain standards, and implement change in respect of this.

- To act at all times in accordance with the Nursing & Midwifery Council (NMC) code of professional Conduct or relevant professional body.
- To perform all aspects of the role in accordance with Trust policies and other national bodies.
- To participate in the assessment of care needs and the development implementation and evaluation of individual care plans.
- To provide specialist advice to other members of staff and act as a role model.
