The role will require the candidate to work across multiply sites which include GEH, SWFT Community North Warwickshire, and the HF Virtual ward. Management and leadership of the Heart Failure (HF) nursing team.
The role will include reviewing and assessing HF patients. Communicating and liaising with MDT, GPs, HCPs, HF patients and carers. Recommending evidence based therapies which include medications, Cardiac rehabilitation and cardiac devices. Patients will be reviewed in Face to face and virtual Clinics, virtual ward, at home and via telephone.

Main duties of the job
- Risk-stratify and plan care for all HF referrals received from HF team, in-hospital, GP Practices, HCPs, self-referrals.
- Provide highly specialised, evidence based information regarding all aspects of Heart failure, intervention and treatments.
- Manage the telephone calls, clinic and virtual consultations to support HF patients ensuring optimisation of all therapies.
- Develop care plans with shared decision making between individuals and professionals. Plans should endeavour to address depression, anxiety, loneliness, as well as healthy behaviours.
- Independently manage and run HFSN led clinics Face to Face or Virtual, providing information and advice regarding explanation of diagnosis, symptom control, risk factor management, stress management, exercise, dietary and HF medicines advice and HF medicines optimisation.
- Virtual ward assessing patients, ensuring HF patient have monitoring equipment, medications, Cannular, IV furosemide. Daily ward rounds assessing effectiveness of IV furosemide and implementing clinical plan.
- Virtual ward discharging patients ensuring patient clinical safe, on-going referrals are in place and communicated to HCPs inclusive of GP.
- Make/receive direct referrals to and from relevant members of the multi-disciplinary teams and Primary Care colleagues when appropriate.
- Initiate investigations and advice on appropriate treatment.
- To prescribe as a non-medical prescriber appropriate medical therapies as agreed by the trust.
