Main duties of the job
An opportunity has become available to work in the Complex Discharge team, covering maternity on a 12 month secondment. This is an exciting role for an experienced band 5 or Band 6 Nurse/Allied Health Professional to work in the Complex discharge team. This role will provide you with the knowledge and skills to facilitate discharges for patients who no longer require acute medical intervention. We support patients and their carer/families in providing the information to assist in their discharge from hospital. A long term decision should never be made in hospital and everyone is entitled to recover from their illness/ injury. Therefore we ensure patients understand the pathways they are being supported on, we also share our base with the Hospital Social Care Team and Arden Mental Health Acute Team. This role is 8-4 Monday- Friday and work at least 1 in 6 weekend shift. You will also be required to work at least 1 bank holiday shift (excluding Christmas day, boxing day and new years day).

Clinical Practice1.1 To support the ward teams in the identification of patients with complicated dischargeplanning needs at an early stage in order to prevent avoidable delays and minimise thepotential for conflict to arise.1.2 Provide specialist assessment for patients with complicated discharge needs, with dueconsideration of their human rights with regard to Mental Capacity Act, Protection ofVulnerable Adults, Deprivation of Liberty, Independent Mental Capacity Advocate.1.3 Support the Ward Managers to ensure that all patients have a clearly documented andaccurate plan of care for discharge or transfer of care, which reflects the assessmentundertaken and incorporates the issues and recommendations made.1.4 Co-ordinate a seamless service through the development of enhanced Multidisciplinaryteam (MDT) processes and communication.1.5 Attend and ensure that all patients with complicated discharge needs that are on thecaseload are discussed at the weekly / regular MDT meeting.1.6 Attend multidisciplinary team meetings and case conferences as appropriate.1.7 Where appropriate support the Continuing NHS Healthcare assessment process, forexample attend Continuing Health Care panel meetings, if required.1.8 Maintain strong links with local community, social services, care homes, housing andvoluntary organisations in order to develop ongoing working relationships to ensure qualityand minimise risks for patents.
