An exciting opportunity has arisen within Medicine for a Practice Facilitator to work across our Care of the Elderly and Stoke Rehabilitation wards.
This post will work in partnership with the Practice Education & Development team to provide support and guidance to staff across both clinical areas.

Main duties of the job
The post holder will work in partnership with the Practice Education & Development team to provide support and guidance to staff across the medical wards. They will provide practical and pastoral support to staff, escalating any concerns to the Ward Managers and Practice EducationTeam as required. They will support staff in the development of competence and confidence in the delivery of high standards of care whilst being responsive to the needs of patients, relatives and carersas well as service demands. In addition, they will ensure that appropriate learning environments are created and maintained to drive the integration of theory with practice.
