Lead Pharmacy Technician - Medical Division

Job summary

The post holder will work closely with the Lead Pharmacist forMedicalDivisiontoprovide a comprehensive wardbased,clinical serviceto theMedical Division.

The role will include taking medicines histories and completing medicines reconciliation, carrying out therapeutic drug monitoring, making therapeutic interventions within scope of practice, supply of medicines, final checking of medicines and counselling patients. In additionmanagement responsibilities will include line management of junior staff,assistingwith the day to day running of the service and monitoring performance, of the service.The role will also involve making continuous improvements to the service using innovative techniques to develop the technician role and integrating completely in the multi-disciplinary team. There will also be operational commitment to the Pharmacy service weekends and Bank Holidays on a formal rota (currently1:6).(ROTATION FREQUENCY SUBJECT TO CHANGE)

Main duties of the job

Clinical Responsibilities:

  • To work towards and complete the level 4 Clinical Diploma for Pharmacy Technicians.

  • To ensure that medicines reconciliation is completedin a timely mannerby communicating with patients, carers, GPs to ensurean accuraterecord of medicines aiming to resolve any discrepancies or issues.

  • To ensure that patient's medicines are availablein a timely mannerduring their inpatient stay by checking Patients' Own Drugs and completing medication orders.

  • To follow Trust procedures where special dosing regimens/medications arerequiredand ensure the nursing staff are aware of any specialist modification wherebyadditionalinterventions may berequirede.g. Parkinson's medication liaising with ward staff at handover.

  • To ensure medicines are made available fortimelydischarge, ensuring that patients and carers have received adequate counselling on their medicines and any changes made to their medicines during their inpatient stay.

  • To complete adequate transfer of care information to Carers, GPs, Care Homes, Domiciliary Care teams, Community Pharmacies and to communicate clearly where clinical follow up isrequiredasstatedin the electronic discharge record.

  • To train and support pharmacy and ward staff to work to a high clinical standard, making patient safety and wellbeing their priority.

  • Accuracy Checking Technician Qualification
  • Achieved Accredited MM/Medicines Optimisation qualification or willingness to complete if not already achieved
