We are excited to introduce a brand new opportunity for two End Of Life Care Facilitators to join our fantastic team in Herefordshire. The Specialist Palliative Care Team is a well established team of two doctors and Clinical Nurse Specialists working across community and acute settings. These roles will work alongside this supportive team.
These pivotal roles are for someone who is driven by compassionate person centred care and wants to shape how EOLC is experienced across acute and community services.
As our EOLCF you will be at the heart of improving quality, consistency and confidence in EOLC. Working collaboratively with health and social care professionals, providers, families and stakeholders. You will support best practice, drive improvement and and help ensure people are able to live well right up to the end of life in the place that matters most to them. Some of this work will be done by being in embedding the Gold Standards Framework model across Wye Valley Trust.
These are newly created roles offering a genuine opportunity to influence, innovate, and leave a lasting legacy in a county known for its community values and collaborative working.
Main duties of the job
You will combine strategic thinking with hands on support, education and relationship building making the role ideal for someone who thrives on both leadership and meaningful human connection.
You will do this by expert clinical knowledge and effective communication and strategic decision making. You will focus on patient safety, staff wellbeing and continuous quality improvement to shape future services as part of the ICB palliative and EOLC strategy.
You will collaborate with colleagues in WVT , Integrated Care System and beyond to actively support and facilitate implementation of the GSF model within HCH and associate community settings. You will promote choice, comfort and dignity for those in the last year of life and their families. You will be able to promote best practice in Advanced Care Planning, facilitating compassionate and informative conversations and enabling discharge to preferred place of care and death.
