Are you interested in an exciting role within the health visiting service? We are looking for an enthusiastic practitioner to join the health visiting service as an Early Years Practitioner.

As an Early Years Practitioner you will be a key member of the health visiting team working across the Hereford City Team. The Early Years Practitioner will support the teams by undertaking clerical/clinical duties as directed to support the delivery of the health visiting service to children, young people and families.

The post holder will work under the direction of the skill mix team including Practice Leads, Health Visitors and Staff Nurses using their skill and expertise to ensure the safe delivery of the service.

The Early Years Practitioner is responsible for completing appropriate delegated tasks and escalating any concerns or risks.

You must have a full driving licence and have access to a car for work as you will be based within the community.

Main duties of the job

You will be responsible for setting up and clearing up from health visiting clinics, and carrying out duties during these sessions such as weighing and measuring children.

  • To assist as appropriate in setting up and facilitating group work for children and parents in conjunction with the Health Visitor/Community Nursery Nurse (e.g. Antenatal workshops, Weaning Workshops).

To set up and facilitate pre planned community workshops within PCN areas (e.g. vaping/smoking, healthy eating etc.). This will include exploring and booking appropriate spaces within the PCN area, creating information boards and collecting resources for parents and delivering a pre-panned workshop which will include presenting information to parents in groups.

To organise promotion displays as delegated by the Health Visitor e.g. ordering, maintenance and collecting of resources.

Provide practical support to parents when caring for their child, whilst promoting safety in the home setting.

Provide evidence based support, assistance and advice to mums who are breastfeeding or plan to breastfeed.

Support mums who chose to artificially feed their babies in line with local and national policy.

Under instruction of the Health Visitor in the community setting visit families in their home and complete assessments of the baby (e.g. growth monitoring, feeding) and report or escalate any concerns you may have to the Health Visitor.
