Job summary
This is an exciting opportunity to join our well established Community Services in a brand new role. we are looking for 2 dynamic and motivated individuals to join us by working in an integrated way across multiple community services. We provide healthcare and support for patients in their own homes to offer a safe and effective alternative to hospital admission, and teams that are proactively keeping people well.
It is a community role when you will see patient sin their own homes, using your own car for transportation. you will help coordinate the patients care and be an integral part of their care provision.
Main duties of the job
Working alongside GPs, ACPs, Nurses and therapists to support the assessment of patients in their own homes, ensuring they can access the healthcare they need. this will include taking bloods and basic observations to support their medical assessments. You may be visiting patients on your own and will ned to feedback your assessments to the wider team. Or you may be visiting with others to support their assessments.
Working as a community falls responder to use moving and handling equipment to return people to a place of safety after a falls,
Assessing for and providing equipment like walking aids and toilet frames if required.
Supporting the day to day sunning of the teams and services you work within.
Providing basic care and supporting personal hygiene/toileting/ meal preparation for patients in crisis and requiring enhanced support to prevent them going into hospital.
