Hepatology Nurse Specialist

Job summary

We are looking for a motivated, dynamic person, with a genuine interest in liver disease to join the Hepatology team. The successful candidate will be part of the Wye Valley NHS Trust Hepatology Nursing Team. Training will be provided. Continued peer support is essential to this role and as a team we are committed to provide this.

The successful candidate must have an interest in delivering high quality, evidence-based care to patients with chronic liver conditions in both the hospital and community settings. Although team working is key, the successful candidate will be expected to work independently , prioritising workload in a variety of clinical settings to meet the fluctuating demands of this role.

You must be self-motivating, enthusiastic, organised, computer literate and able to demonstrate excellent interpersonal skills with a commitment to continued personal development. As a member of the multi-disciplinary Hepatology team, the post holder will provide nursing expertise related to liver disease to colleagues across the Trust and community.

Main duties of the job

  • To support the Hepatology CNS and Hepatology Consultants in the management of patients with liver disease, using evidence based/patient centred principles to assess, plan, implement and evaluate routine care.
  • Provide specialist liver disease advice, to patients and their families/ carers, other healthcare professionals and students.
  • Act as a clinical resource for the management of liver disease within the trust, providing expertise that reflects research and current best practice.
  • Take responsibility for own patient caseload/workload within the designated clinical area and for the assessment and treatment of patients on a day to day basis. Escalates concerns to senior Hepatology CNS and Consultant Hepatologist in a timely manner
  • Work within Hepatology outpatient clinics undertaking follow up reviews of patients with chronic liver disease.
  • Following recognised training plan work autonomously to deliver a nurse led Fibroscan clinic
  • Work closely with the multidisciplinary team to provide support, advice and education for patients.
  • To work with the Lead Clinician, Lead CNS, Hepatology CNS and Primary care stakeholders, to deliver clinical pathways and direct access services for patients with acute and chronic liver disease
