Job summary

We have an exciting opportunity for two ACP's to join an innovative and forward thinking team within the Cardiology Department. These roles are an expansion of our ACP workforce and enable the service to expand and develop the scope of both inpatient and elective services.

The aim will be to provide 7 day working with our ACP workforce and to improve front door decision making through increased capacity to in-reach to ED. The department plan to expand 'hot' clinic capacity in order to take action in managing internal referral demand and support admission avoidance as well as supporting patients to the right care and right place.

The Cardiac ACP's will support in the delivery of both inpatient and outpatient activity. They will be required to work across the Cardiology Department: including Coronary Care Unit, Acute-Cardiology Ward (Lugg), Cardiac Cath Lab, Outpatient Department, Same Day Emergency Care (SDEC), Virtual Ward and appropriately referred patients within the Emergency Department.

Band 6-8a dependant on experience. The preference is for candidates who already hold an MSc in Advanced Clinical practice. Consideration will be given for the right candidate to complete the MSc via an apprentice with our team.

Main duties of the job

The ACP role is at the forefront of Cardiology with non-medical practitioner's crossing professional boundaries, undertaking traditional medical roles and leading the way for inter-professional practice. The ACP will be able to assess and treat varied clinical cases, having advanced skills to assess the patient with a differentiated or undifferentiated clinical diagnosis. Advanced communication, problem solving and decision making skills will be used to initially assess and provide on-going patient care.

The post holder will provide clinical expertise for developing and enhancing the service and will demonstrate advanced skills and competencies and ensure that evidence-based practice is inherent in all aspects of care and treatment.

Working within local and nationally agreed protocols and guidelines, the ACP exercises independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of Cardiac patients. They work collaboratively within the multi-professional team and support the education and development of both nursing, medical and allied-health professionals.
