Job summary

Do you have experience in the management and care of patients with liver disease and are you passionate about improving Hepatology care for patients?We are looking for a motivated, dynamic person, with a genuine interest in liver disease to join the Hepatology team.

The successful candidate must have an interest in delivering high quality, evidence-based care to patients with chronic liver conditions. You must be self-motivating, enthusiastic, organised, and able to demonstrate excellent interpersonal skills with a commitment to continued personal development. As a member of the multi-disciplinary Hepatology Team, the post holder will provide nursing expertise related to liver disease to colleagues across the Trust and community.

Main duties of the job

The post holder is to be responsible and accountable for the provision, and continued development of a specialist nursing service for patients with a variety of liver conditions working alongside the wider Hepatology/Gastroenterology team. The post holder will manage a caseload of patients with a variety of chronic liver conditions including cirrhosis; liver steatosis, alcohol related liver disease, autoimmune liver disease and viral hepatitis with support from the multi-disciplinary Hepatology/Gastroenterology team.

Using advanced practice skills and associated expertise the Hepatology CNS will undertake physical assessment and history taking of patients and provide immediate care and/ or initiate treatment as per clinical pathways/ protocols. The Hepatology CNS will undertake non-medical prescribing in line with Wye Valley NHS Trust policies and procedures.The post holder will facilitate nurse led clinics for hepatology patients, including cirrhosis follow-up clinics, abnormal LFT's, viral hepatitis, fatty liver, hepatocellular carcinoma surveillance, and autoimmune liver disease and Fibroscan clinics.

The post holder will be part of the nurse led outpatient elective paracentesis service, independently undertaking once trained, elective paracentesis for ascites in decompensated liver disease.
