We are looking for enthusiastic, dynamic and passionate nurses to join our hugely expanded Frailty service at Wye Valley NHS Trust.
At the end of 2023, we opened our new state of the art frailty unit and massively expanded our acute frailty services to provide expert, dedicated care for frail patients and those with living dementia across Herefordshire. This has now been enhanced further by the opening of our Frailty Same Day Emergency Care Unit in September 2023.
We are looking to employ staff nurses who are dedicated to providing the best possible care for these patients and want to be part of growing and dynamic service. Patients are admitted to FSDEC from A&E with the support of our team of Frailty ACPs, with the goal of admission avoidance where appropriate. They may be discharged home with enhanced support, transferred to a community hospital for rehabilitation or to one of our specialist frailty wards for further acute management.
Main duties of the job
To provide patient centred care as part of a multi-disciplinary team.
To champion excellent holistic care for frail older people.
To support and supervise new members of staff and students.
On agreement, to accept occasional responsibility for the management /leadership of the ward department in the absence of senior nurse.
To facilitate a safe working environment by ensuring Risk assessment updated and robust documentation is completed.
To contribute to service improvement projects as required to support better care.
