This post holder will work within the ward-based pharmacy team providing a pharmaceutical care service within the medical and surgical directorates based at Hereford County Hospital.
The role is to be directly involved in the delivery of high quality care to patients at ward level through efficient medicines optimisation. This will include the assessment of patients' own drugs, medicines reconciliation, discharge-planning, counselling of patients about their medication and coordinating the supply of discharge medication to support efficient patient flow throughout the hospital.
Main duties of the job
Clinical Responsibilities:
- To confirm and document medication histories for patients brought into the acute wards covered by the service in line with relevant procedures.
- To communicate with patients, carers, GPs and community pharmacies to ensure that medication histories are accurate.
- To assess patients' own drugs in line with relevant procedures and to ensure that the drugs are stored safely following the appropriate standard operating procedures.
- To ensure that patients' own drugs are transferred with patients either at the point of transfer to another ward or at discharge.
- To facilitate timely discharges from ward areas by completing discharge prescriptions efficiently when required.
- To communicate with patients and nursing staff to ensure timely supply of medication especially critical medication.
- To communicate drug and medication related information to patients, carers, relatives and members of the multidisciplinary team and provide counselling where necessary.
- To discuss optimal compliance solutions with patients and carers with special circumstances.
- To provide advice to patients and carers regarding doses and side effects to optimise compliance.
- To use knowledge and experience to ensure that prescribing falls within the WVT formulary and refer any issues outside of their remit to the pharmacist responsible for the area in question.
