Job summary

An exciting opportunity has arisen to join us as a Macmillan Information Facilitator based in the Macmillan-Renton Cancer Unit, at Hereford County Hospital. This role involves the following:

  • In conjunction with the other team members of the Macmillan Cancer Information and Support Service (CISS), be responsible for implementing a comprehensive information service to ensure accessibility and equality of information and support for all people affected by cancer living in Herefordshire and Powys.
  • To work with the Macmillan CISS team members, service users, clinicians, cancer unit teams and the voluntary sector to meet the broader needs of cancer patients in terms of information and support.
  • The day to day management of the Cancer Support Worker team in maintaining a strong 'living with & beyond cancer' support service.

Main duties of the job

  • To provide timely and individualised information and support to patients and relatives, carers and members of the multi-professional team. This may be in person, by phone, email or letter
  • To provide and maintain a high profile service within WVT and the surrounding community localities.
  • To contribute to the development of clinical protocols, guidelines and procedures in conjunction with other members of the multi-professional team to address the information and support needs of patients, carers and Healthcare Professionals
  • To ensure involvement with the local patient support groups and other voluntary organisations on the provision of accessible and acceptable cancer information, ensuring their involvement in the development and provision of the Macmillan CISS
  • To maintain accurate and up to date records on uptake of the service.
  • To act as an expert resource of cancer related information.
  • To contribute to the development and implementation of policies, procedures and guidelines on patient information and support across Herefordshire.
  • Liaise with health professionals and other key stakeholders to promote and raise the profile.
  • To maintain confidentiality of service users where appropriate.
