Job summary

We are offering the exciting opportunity for two youth workers to support children and young adults, from 11-25 years, as they develop independence in managing, not only their medical conditions, but their emotional wellbeing, opportunities and individual ambitions as part of our diabetes and epilepsy services at The County Hospital, Hereford.

You will need to be adaptable, identifying the differing needs of adolescents and young adults as they pass through life's transitions into adulthood.

You will work within our clinical teams bringing to the table your expertise and experience in patient advocacy and a voice to our children young people. You will shape the service toward a more child/young person centred specification, advising on and developing initiatives and a network of resources that will help our children become confident young adults.

You will receive guidance and support from a senior youth worker and the clinical teams, as well as supervision and peer support from external mentors and forums. We want you to be able to develop your own ideas in promoting advocacy and wellbeing and have the confidence to implement them.

Main duties of the job

  • To develop a high standard of patient advocacy for children and young people within the diabetes and epilepsy services based at The County Hospital, Wye Valley NHS Trus
  • To act as an advocate and knowledgeable source of support for children and young people within the paediatric and young adult diabetes and epilepsy service
  • To support young people through transitioning from paediatric clinical care to young adult clinical care, up to 25 years of ag
  • To work flexibly to assist with the provision of a varied programme of personal, social and educational developmental activities with due regard for the needs and opinions of young people. This could include evenings, weekends and residential work
  • To participate as a professional member of the Youth Service and contribute to the care of the patients of the Trust, relating to their all-round cultural and diversity, developmental and emotional needs.
  • To contribute to the development of the Youth Service through identified project aims communicating effectively with the Senior Youth Worker Project Manager and the Paediatric and Adult Specialist Teams
  • To support and advice the specialist teams of vulnerable children and young adults and contribute to problem-solving strategies to support the
  • To assess the social, developmental and educational needs of individuals and groups of young people and help deliver appropriate programmes and activities.

Essential

  • Youth Work Diploma/Degree or professional youth work qualification
  • Equivalent in youth work, psychology or Social Sciences (Dip HE /JNC level) Or extensive experience working with children and young adults in an advocacy role

  • Evidence of continued professional development

Skills, Knowledge & Abilities

  • Good understanding of the purpose of Youth Work within a hospital setting.
  • Knowledge, experience and application of current legislation and guidance governing the delivery of youth work services. Knowledge and understanding of issues relating to child protection and cultural and diversity awareness.
  • Organisational and time management skills, able to work on own initiative
  • Knowledge of issues relating to consent and confidentiality.
