Job summary

An exciting position is available to join the Inflammatory Bowel Disease (IBD) Team as a Nurse Specialist.

You will provide expert support and advice for people living with Inflammatory Bowel Disease on a daily basis to enable patients to have the best quality of life possible. Achieved through high quality Nursing care and Multi-Disciplinary working.

You will work autonomously and within a multidisciplinary team to provide individualised, timely, holistic care through nurse-led clinics, the IBD advice line and by supporting inpatient care. Enabling patients access to high quality clinical care; meeting the IBD Standards and enhancing the patient experience. You will play an integral part in the weekly IBD case review meetings with the consultants and monthly IBD MDT's. You will provide clinical leadership assisting in developing and improving the IBD service and supporting other members of the team.

Main duties of the job

You will be responsible for communicating highly sensitive information to patients and carers in an empathetic and supportive manner. You will be able to interpret results and empower patients to manage their condition with best practice, to include medication counselling and titration. You will be able to identify patients who require additional support or investigation and will escalate their care appropriately.

The successful applicant will be expected to support newly diagnosed and existing IBD patients who may have complex needs, working as an advocate and relaying accurate information. Being a good communicator and accurate record keeping is vital in this role.

The role includes audit, service development and formal education /teaching, to include supporting the local Crohn's and Colitis UK group.
