Senior Discharge Facilitator

Job summary
To lead on coordinating the discharge or transfer arrangements of medically and therapeutically stable patients identified as medically fit for discharge within a designated area. The post-holder will participate in the assessment of after care needs of clients and relatives/carers, contacting where necessary other members of the multidisciplinary team.
As part of the D2A team, work in close liaison with members of all areas across the hospital ensuring systems are in place for the effective flow of patients through the hospital aiming to provide a high quality service and meet Trust targets.
To collate and provide up to date capacity/bed information to support patient flow to the Manager Team and on-call managers with regard to Rehabilitation wards.
Main duties of the job
To ensure patient's needs are addressed and coordinated in the most timely and effective way.
To act as a link, liaising with other agencies and departments, to provide efficient and effective communication to expedite progress towards there onward care.
Develop working relationships with ward teams to enable them to support and co-ordinate flow across the West park hospital and Cannock (Fairoak) sites. .
In collaboration with clinical teams, health and social care agencies, Senior Discharge Facilitator will support the Discharge Facilitators and Discharge to Assess Team in planning and managing the daily flow through West Park Hospital and Cannock sites.
Will contribute to the identification of resources and service delivery in order to facilitate the appropriate and timely discharge of patients.
