School Nurse - Single Point of Access (0-19 Service)

Job summary

We have an exciting opportunity for a Band 6 School Nurse to join the School Nurse Single Point of Access (SPoA) (0-19 Service).

As a school nurse working within the SPoA you will work within a small team to manage and co-ordinate the day to day referrals into the service, triaging referrals, managing telephone calls and supporting with safeguarding, including attending safeguarding meetings.

The post holder will be required to have NMC registration.

The post holder will contribute to the delivery of the organisation's objectives.

This position is part time working 18.75 hours per week all year round.

Main duties of the job

To triage and manage referrals into the School Nursing Service within an agreed timeframe and forwarded onto teams as required.

To support with health care plans.

To manage the Paediatric Sleep Pathway with initial assessment and support with sleep diaries.

To support to ChatHealth Service.

Supporting the Families First for Children Pathfinder Programme (FFCP) with attendance at meetings and contribution to reports and information requests.

To co-ordinate movements in and out.

Signposting.

Health Promotion and parenting advice and support.
