Main duties of the job

The Discharge to Assess (D2A) Team are a nurse-led team coordinating discharge of patients with complex health needs. The team receive referrals from the acute setting and work collaboratively with Social Care and community therapy services to identify an appropriate discharge pathway.

Taking a 'Home First' approach we help provide patients with reablement at home or short-term care and 'stepdown' beds to bridge the gap between hospital and home to facilitate assessments of ongoing health and social care needs. Wherever possible, patients should be supported to return to their home for assessment.

We are looking for an enthusiastic, pro-active band 6 nurse with a keen interest in hospital discharge to join our team. You will need excellent communication and interpersonal skills working with the patient, families, and the multi-disciplinary team to coordinate safe, timely discharges. In addition to the above, as a Junior Sister within the Care Coordination Team you will be required to provide managerial leadership support to the team.
