Job summary

We are now looking for a Specialist Nurse Practitioner to join the Care Coordination Team. We are a service that operates 24 hours a day, 7 days a week and want to expand on the work we do in ensuring patients in the community access the services they need. We support emergency services in directing patients to the right place of care and supporting our acute colleagues to discharge patients in an appropriate timeframe - ensuring continuity of care is delivered at home as required.

You must hold a full UK driving license and have access to a car for work purposes.

Main duties of the job

The post holder will be required to have experience in triage and care of the acutely ill adult, demonstrating competency in both rapid and holistic triage to ensure patients are signposted and referred to the correct service to meet their needs. Due to the nature of the role you must be confident in completing telephone triage and utilizing web based applications to complete video triage and remotely monitor patients. You must be IT literate and be able to work at a fast pace to meet demand.

The post holder will be required to complete face to face assessments for patients in their homes or an acute setting when required and therefore must be competent in elements of care delivery as required of a Specialist Nurse Practitioner. The post holder must have health assessment and prescribing qualifications.
